Aegis Access Network

Aegis Access Network (AAN) provides support coordination (case management) to our
clients by assisting with accessing services and resources needed for each client’s optimal
quality of life. AAN can coordinate community services such as: medical care, access to
appropriate doctors, client’s prescribed medication, therapies, equipment, personal, social, and educational services.

About AAN

Aegis Access Network is an approved and licensed provider with the Louisiana Department of Health to provide Case Management (Support Coordination) in Region 1 which include: Orleans, Jefferson, St. Bernard, + Plaquemines Parishes. We are also a certified Medicaid Application Center where we assist individuals and families with enrolling in Healthy Louisiana Insurance Plans.

We value each of our clients, so it is our top priority to provide exceptional care and facilitation. We are dedicated to building partnerships with individuals, their families, and all support team members to assist in each client’s life goal fulfillments and community productivity.

Services

Support coordinators will assist our clients by developing a full list of all the services and resources needed by the client, then helping the client to obtain them. This can include things like medical care, access to the client’s doctor prescribed medication, therapies, personal care services, equipment, personal, social, and educational services.


Support coordination will develop quality person centered plans and monitor plan implementation to assure personal outcomes are met. Support coordinators will provide information and assistance to individuals in directing and managing their services.

Our services are rendered without discrimination regarding race, religion, sex, age, nationality, economic and social status. The client is not obligated to receive services from this agency. We operate solely by freedom of choice and the client’s choice does not affect eligibility for wavier, state plan, or support coordination services.

  • Linkage to supports and services
  •  Flexibility to choose options and services that best meet each client’s individual needs
  • Access to a Case Manager (Support Coordinator) to assist in developing a
    Comprehensive Plan of Care (CPOC)
  • Client Advocacy
  • Maintain phone, virtual, and face-to-face contact with the client and their support team, as well as providers to ensure that the CPOC and services are being delivered appropriately
  • Progress Monitoring of the client’s CPOC for effectiveness of services rendered by
    providers

Who We Service

EPSDT (Early and Periodic Screening, Diagnostic and Treatment) eligible participants
receive complete and preventative health care management of services, Medicaid and
non-Medicaid, which are key to ensuring that children and youth under the age of 21
receive appropriate and timely dental, mental, health, developmental and specialty
services.

OCDD (Office for Citizens with Developmental Disabilities) waiver eligible participants
which include Children’s Choice Waiver, Supports Waiver, Residential Options Waiver
and New Opportunities Waiver will receive assistance in gaining access to needed
medical, social, educational and other support services by the development and
coordination of an individualized plan of connections to community resources, and
monitoring of plan implementation to assure personal outcomes are met.

Self Pay

Self-Pay option is for persons who would like our case managers to navigate the difficult world of needed services and providers for our clients’ optimal life goals and care.
